Failed to initialize audio device warning on .flv files

In latest fw 1.5.0.23643, may users including me experiencing Failed to initialize audio device warning pop up on HDMI out. I've experienced it on .flv file with mp3 audio & some other users experienced it on different format video/audio files as well. Its not directly related to http://jira.boxee.tv/browse/BOXEE-12522 as in that issue user forgets to turn on AVR and the warning pops up, but in my case everything is on and I still get the warning. Then I've to reboot BB to get audio back :( I'll post the file info shortly. Have a look at this thread http://forums.boxee.tv/showthread.php?t=53366&page=2 to know what other users are experiencing.
